DVLA forms for conversions

Hi All,
over the last few months we've seen several very detailed forms shown here for apply to DVLA to change vehicle classification to a campervan. I've just received a form from my insurer which looks a heck of a lot simpler. I'm going to get it completed by my local dealer and see what DVLA's reaction is to this particular form. Watch this space.
